The Questionnaire Calculation Sheet calculates "scores" for questions, groups of questions, and the questionnaire overall, one response at a time. It is specifically designed to score an individual response. For example, a report could be produced scoring each individual employees satisfaction with their job. If there were 100 employees, 100 individual "sub-reports" would be produced.

The key element of any survey that could use a calculation sheet report is that it must have a number of ratings questions (from which the calculations will be derived). In addition, these questions should belong to certain "groups". For example, an employee satisfaction survey might have a group of questions that all relate to work environment. A calculation sheet could then be produced that shows scores for the different "groups", and scores for the questionnaire overall.

This historical rating capability of this report can be used when you rate the same company, person, store, division etc. repeatedly over time. Note that each questionnaire must have a common answer to a particular question (eg. Company Name) so that multiple responses can be grouped. The report would show the results for the most recent response, together with historical ratings. This method of analysis is only applicable in cases where a single response is entered for each company, person, store etc. in each time period. Consider the following example:

Munklecorp performs a customer satisfaction review every quarter. Each client is sent a survey each, and a single response is therefore entered each quarter per company. The questionnaire calculation sheet is produced each quarter - one report per company. The report shows each company's most recent response, together with the historical responses from previous quarters. This provides a clear indication of whether client satisfaction is improving or worsening, on a client-by-client basis.

What can I use this report for?
The Questionnaire Calculation Sheet is very useful in situations where you want to consolidate a number of ratings into a single percentage. It makes comparisons of ratings very simple because all ratings are "distilled" into a small number of percentages (generally one for each area rated, and an overall rating). Uses of this report include:

To individually rate responses, and, where applicable, show historical ratings for related responses.

To "distil" a large number of ratings in various areas down to a number of overall percentage ratings.
